and Feedback Forms These are the three forms that you find at
the end of every printed booklet. They can be used to
provide feedback and assessment about the Learning Unit.
Learners assess their own and their peers progress and
Educators evaluate and provide feedback about the Learning
Unit.
Printable Version: Feedback and Assessment Forms
This is a printable version of the forms. You can print
these out and photocopy them. These forms must then
be filled in manually with a pen or pencil.
Integrated
Module Feedback Form Several of these forms are usually found in
each of the Volume white files containing your printed
booklets for each Grade. They are a means of providing
feedback to our development team about your views on
the Integrated Learning Units and our product as a whole.
Positive as well as any negative comments would be greatly
appreciated! They should be completed after each lesson
and forwarded to Head Office at the end of each term.
Integrated Assessment
Pages
The Integrated Assessment pages are those found on page
4 in each of the printed booklets. It is a matrix which
can be used as a means of assessing the skills and outcomes
being developed in the lessons. These documents are
available in MS Word formats.
